چکیده
Introduction: Breakthrough Pain (BTP) is experienced as mild to moderate-severe pain, from only a few seconds hours. It causes decrease in the quality of life and functional capacities. Furthermore, BPT must be recognizable, assessed, controlled prevent its relapse severity. The patient had pain score NRS 9, which was felt intermittently for last 3 months. Treatment has been carried out MST 10 mg/8 hours Durogesic® patch (fentanyl 50 mcg/h) but did not subside. Moreover, unable identify any precipitating factors or relievers, while diagnosis confirmed BTP. rescue dose administered range – 20% total daily 24 equivalent 11 22 mg intravenous Morphine equianalgesic 110 220 mcg fentanyl. Multimodal Pain Management Protocol Versus Patient Controlled Narcotic Analgesia for Postoperative Pain Control after Shoulder Arthroplasty
Background: Our institution’s traditional pain management strategy after shoulder arthroplasty has involved the utilization of postoperative patient-controlled narcotic analgesia. More recently, we have implemented a protocol (TLC) that utilizes a multimodal approach. The purpose of this study was to determine whether this change has improved pain control and decreased narcotic utilization. ...

Purpose: Patients may control postoperative pain by self-administration of intravenous opioids using devices designed for this purpose (patient controlled analgesia or PCA). This study set out to determine whether any of the two opioid administrations (i.e. PCA or conventional analgesia) would provide superior pain relief among patients undergoing laparoscopic cholecystectomy or not. ...

OBJECTIVE Intrathecal therapy (ITT) via an implanted pump has become an accepted practice for the treatment of refractory cancer pain by infusing opioids and adjuncts directly to the neuraxis. Until recently, only a programmed basal rate of infusion could be delivered, and therefore, breakthrough pain required ongoing use of oral or transmucosal opioids. Recently, an implanted pump manufactur...
